Identify Your Priorities

We'll discuss your wants and desires, your values and priorities. We'll also review any documents you currently have. Using our exclusive checklist, we'll guide you through the documents you need to establish a tailored Life Care Plan.

Together, we'll either review or create any advance directives you may need. Steps we take may include:

  • appoint a health care proxy

  • create an Advanced Medical Directive or Living Will

  • complete a POST (physician orders for scope of treatment)

  • document your preferences using the Five Wishes form
