We'll discuss your wants and desires, your values and priorities. We'll also review any documents you currently have. Using our exclusive checklist, we'll guide you through the documents you need to establish a tailored Life Care Plan.

 NAVIGATE YOUR FUTURE

Together, we'll either review or create any advance directives you may need. Steps we take may include:

  • appoint a health care proxy

  • create an Advanced Medical Directive or Living Will

  • complete a POST (physician orders for scope of treatment)

  • document your preferences using the Five Wishes form

LIFE CARE PLANNING: PROTECT YOUR FUTURE

If you choose to continue with your Life Care Plan based on the results of the checklist, you will decide which items to work through.


Some examples of items are:

  • Organize life documents such as wills and trusts, health care directives, investment and retirement plans, insurance, and birth and marital documents

  • Plan for the future by understanding the costs involved in eldercare and what is covered by Medicare, Medicaid and Long Term Care Insurance. We'll explain what financial options you can utilize.

  • Gather all online and financial information in one secure location, like bank accounts, insurance plans, and logins.

  • Make advanced arrangements for funerals and memorials.

As we identify needs, we will refer you to trusted professionals who can support your goals. 

We will make recommendations for trustworthy and affordable:

  • elder law attorneys

  • home care agencies

  • financial planners

  • retirement communities

  • aging life care managers


In addition, Colleen Duewel is an Advanced Professional Member of the Aging Life Care Association and can refer you to a certified, experienced, and compassionate Geriatric Care Manager (GCM).
